Back to School: Immunizations Required for Illinois Students - chambanamoms.com

Thank you to Christie Clinic for presenting our 2021 Champaign-Urbana Area Back to School series.

Christie Clinic is encouraging parents to schedule a school physical soon.

Starting school physical appointments now will allow Christie Clinic Department of Pediatrics and Department of Family Medicine to best serve all patients while maintaining a safe visit environment. Enhanced precautions are in place to keep patients and team members safe including additional hours, visitor restrictions and social distancing to reduce the number of people in our facilities.

Parents can currently make back-to-school appointments for their children through their MyChristie patient portal or online at christieclinic.com.

Requirements vary based on what grade your child is entering. Here’s what you’ll need to know about immunizations to make sure your child is ready for the 2021-22 school year.

Note: some schools require that students going into Pre-K, kindergarten, grades 6, 9, and 12 must submit the state required immunization records as listed below before they will be allowed to enter school. Check with your district to be sure of the date.

  • After Sept. 1, 2021, Champaign Unit 4 students not in compliance with the immunization or physical requirements will not be allowed to attend school. Appointment cards will not be accepted so make sure to schedule your child’s appointments with time to spare.
  • Immunization requirements for students in Urbana School District 116 are due at the start of the school  year, and must be submitted no later than Oct. 15, 2021.
  • In Mahomet-Seymour schools, all Pre-K, Kindergarten, 6th and 9th grade students must submit required immunization records as listed below before they will be allowed to enter school.

Vaccinations

The State of Illinois requires vaccinations to protect children from a variety of diseases before they can enter school. Students must show proof of immunization against up to 12 vaccine-preventable diseases (the number and schedule of these vaccinations depend on a student’s grade and age).

Exemptions to immunization requirements:

  • Religious: Parents/Guardians requesting religious exemptions from health requirements must complete the required form along with their child’s healthcare provider.
  • Medical: If your child has a physical condition that prevents adherence to the vaccination schedule, their healthcare provider should indicate this on a physical examination form or in written documentation. Depending on your child’s medical condition, this may need to be reviewed on an annual basis.
